• Bank statement or photocopy of bank book for the last 6 months (2 copies)

• Post dated checks for down payment & monthly amortization

For locally employed:

• Certificate of employment & compensation (original & photocopy, 2 copies)

• Latest Income Tax Return (2 copies)

• Income Tax Return for the last 2 years (2copies)

• Financial Statements for the last 2 years (2copies)

• Business registration (2copies)

• Mayor's permit (authenticated, 2 copies)

For OFWs:

• Latest contract of employment (authenticated by the POEA, 2 copies)

• Certificate of employment & compensation from agency for Seaman only (original & photocopy, 2 copies)

• Income Tax Return (2 copies)

• Photocopy of Passport (2 copies)

• Special Power of Attorney
  Exit Pass, for Seaman only (authenticated by the POEA, 2 copies)

SPA:

• ID Pictures (2 copies)

• Residence Certificate ID (photocopy, 2 copies)
